Once the order form has been printed, fill in your name and address and select the books you wish to order. Multiply the quantity of each print by the cost of the print and write the extended price of each print to be ordered. Add the prices in the last column for the Total Cost of Prints at the bottom of the list. 3. Add 5% Sales Tax. Find the Shipping cost in the chart below that applies to the state where the prints will be shipped. Enter the amount for any Additional Costs that may apply. Add the Total Cost of Prints, Sales Tax, Shipping, and Additional Costs for the Total. 4. Select you method of payment. Mail, phone or fax your order. Please use your account number, if one has been established, when placing orders. To insure you are shipped the correct prints, please order by both print number and print title. Prints are generally shipped within 1 to 4 days. Measurements: Sizes within catalog and price sheet are exterior sizes and are rounded off to the nearest quarter inch. Terms & Conditions: Payment may be made by MasterCard or Visa, or orders may be shipped COD ($7.00 COD charge). Minimum Credit Card Order $15.00. For customers with approved credit, our terms are NET/30 from shipping date. A finance charge of 1.5% may be charged to past due accounts and account will be changed to COD. Customer is responsible for all costs incurred by the Dietz Press in collecting past due accounts. Returns: Please do not accept damaged packages. Out prints are guaranteed to meet your satisfaction. Any print may be returned within 30 days of receipt. When returning prints, please include a note stating the reason for return, your account number and specify whether you desire a replacement, credit or refund. Please package returns carefully. We will not accept returns that arrive damaged due to poor packaging. Foreign Customers: Foreign customers must pay with US funds in advance, with a certified bank check, by wire, or by MasterCard or Visa. Please call for shipping costs. Shipping Costs: Find your state below. Next day, 2nd day and 3rd day shipping are the same for all states except Alaska and Hawaii.
